I understand that participating in activities at The Slime Lab, including but not limited to slime-making, crafting, handling materials, and using equipment, involves certain risks. These risks may include skin irritation, allergic reactions, staining of clothing, or minor injuries such as slipping or contact with equipment.

I acknowledge that all materials used at The Slime Lab are non-toxic, but I understand that reactions can still occur depending on individual sensitivities.

I voluntarily assume all risks associated with participating in these activities.

In consideration of being allowed to participate in activities at The Slime Lab, I hereby release, waive, discharge, and hold harmless The Slime Lab, its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may occur while on the premises or participating in any activity.

I understand that The Slime Lab is not responsible for lost or damaged personal items, including clothing, electronics, or accessories.

In the event of illness or injury, I authorize The Slime Lab staff to obtain emergency medical care if necessary. I accept full financial responsibility for any medical treatment required.

I understand that photos, videos, and audio recordings may be taken during activities, sessions, or events at The Slime Lab.

By signing this waiver, I:

• Grant permission for myself/my child to be photographed, video recorded, or audio recorded
• Authorize The Slime Lab to use such media for advertising, marketing, social media, and promotional purposes
• Understand that I will not receive compensation for their use

If I do NOT want myself or my child included in photos or recordings, I agree to notify a staff member BEFORE the session begins.

I agree to follow all instructions and safety guidelines provided by The Slime Lab staff. Failure to follow rules may result in removal from the activity without refund.
